Top 10 World’s Catastrophic Accidents in Oil and Gas Industry

Much offshore gas and oil exploration takes place many miles beneath the ocean. As well as involving some seriously complex equipment, oil platforms can also be dangerous places, with numerous environmental hazards to deal with. Well drilling has taken place for thousands of years, with the earliest known instance being in China in 347 AD, and since then there have been numerous accidents which have had a serious impact both on human life and the environment. There is a positive side to this, though- virtually every accident has led to safety reviews, and helped to improve safety protocols and equipment.

Below, we’ve put together a list of the ten catastrophic oil rig accidents, to demonstrate just how dangerous this industry can be.

Perro Negro 6 Jack Up Rig Capsized and Sank – Oilfield Safety

Perro Negro 6 Incident – Jack up rig capsized and sank was happen few year ago but we would like to share this case as a case study. You can watch what happen in the video below.

It is clearly seen that the rig capsized and sank in just few minutes. Jack up rig move is one of the most hazardous operation. Before a rig is moved, total number of people on board will be kept at minimum for operating the operation and non essential personnel will not be allowed on the rig.   There are several considerations that rig contractors and operators must be agreed before commencing any rig move operation. For this case, we would like to emphasize that even though all safety is in place, the bad thing can be happened.

Offshore Crane Failure Case Study

Offloading equipment from a supply boat is one of the normal lifting operations for offshore rig. Therefore, it is very imperative to follow safety work practices while working with the crane. Crane operators have the responsibility to work with the crane safely. Before any lifting operation, a pre job safety meeting with all crew must be conducted and the plan must be clearly explained to the crew. A banksman will be only one dedicated people to give the signal to the crane operator. While lifting, there must be no one underneath the crane because anything can happen even though a crane is properly inspected.

This video is very good example to learn about the offshore crane failure. The operation was to lift the subsea well head off the boat to the rig. The crane operator picked up the wellhead from the boat. Unfortunately, the crane did not properly function and it resulted in failure. The crane bloom was damaged and fell into the sea. We strongly believe that nobody got hurts from this situation.

What was happened? Kelly was bent.

We just got photo from our friend sharing these images of Kelly which was bent from parted drillstring.
This is the details.

While working the stuck drillstem using the kelly rig system, maximum tensile load applied on surface was about 375 Klb (170 tonne)  and the over pull was about 155 klb (70 tonne) and then the drill pipe was suddenly parted. This resulted in the Kelly to pop out of rotary table and the swivel bail came out of the hook. Therefore, the Kelly fail down freely because there was nothing to hold it. The equipment went down with the Kelly which was bent due to falling action from the rotary table to outside of the rig floor. Fortunately, nobody got injured from this incident.

Safety First

Safety is one of core values in oil and gas industry. There are a lot of emphasizing on safety while you are in a work place as drilling rigs, production facilities, oil offloading tankers, etc. However, we can see in many situations that safety are not recognized or even considered.  There are several in everyday life where people don’t think about how to work safe or they think these situations are safe in their thoughts. Therefore, we would like to share some images which we would like to use them to raise safety awareness in every day life. Please feel free to share with your friends or colleagues. Moreover, if you have any comments, please feel free to share with us.

Safety is for everyone. Let’s work safe and help people work safe 🙂
